Patient Resources

Valdosta Medical Clinic welcomes new and returning patients. While online scheduling is available to established patients, we ask new patients to complete a short questionnaire as our way of assessing if our practice is a fit for those seeking to become a VMC patient. More on online scheduling:

New Patient

To make sure we are a good fit, we ask those interested in becoming a VMC patient to complete a short questionnaire before we commit to making an appointment. The questionnaire may be accessed by clicking – Patient Application Form (PDF). Once you have completed and submitted this form you will receive a reply within 1 business day. Patients seeking more immediate appointments or physicians/providers not available online are encouraged to call 229.242.8480.

Established Patient

Established patients are welcome to use the online convenience of requesting a follow-up visit with the primary care physician you have seen before. For those selecting an appointment online, you will notice that appointment slots are not available until 2 business days out. This time is needed for our online schedulers to prepare for your visit. You will receive confirmation within one business day of your request. Patients seeking more immediate appointments or physicians/providers not available online, including Walk-in Clinic, are encouraged to call 229.242.8480.

General Information

Online schedule request is not available to patients wanting to change physicians or schedule an appointment with our cardiologist or gastroenterologist. However, appointments with Susan Howell, Dr. Powell’s Nurse Practitioner, are available.

Become a New Patient

We are currently accepting new patients into our practice. Thank you for considering us. To become a new patient:

  1. Make an Appointment
  2. Sign up for our patient portal
  3. Download your patient forms online through the patient portal

When you come to our office for the first time as a new patient, we'll ask you to complete some initial forms, including an Authorization and Consent for Treatment form, if you were not able to download them from the patient portal in advance of your appointment.

To make sure there are no delays in care during your first visit experience, please arrive 15 minutes prior to your scheduled appointment to ensure your registration is complete before meeting with your new provider.

Remember to bring:

  • Your insurance card
  • Valid photo ID
  • List of current medications
  • Office co-pay

In an effort to respect the time of all of patients, our staff strives to stay on schedule so that other patients do not have to wait.

For patients who are delayed and arrive late for appointment, every effort will be made to see them the same day. However, wait times may apply, or appointments may need to be rescheduled.
